Everything worth knowing about your system, on one page
Fill in what you can find. The card fills in beside you, tells you what is still missing, and prints on a single sheet. Nothing is stored and nothing is sent anywhere.
The card
Most of this is printed on a plate on the equipment itself — on the outdoor unit, on the furnace or air handler, or inside the access panel. Photograph the plate while you are there; it saves doing this twice.
On the plate. Usually the longest string of letters and numbers on it.
The one that identifies your particular unit. Warranty claims start here.
If you do not know it, the serial number usually encodes it and we can tell you.
Printed on the edge of the filter you take out, for example 20 × 25 × 1.
Which panel, and which breaker. Worth knowing at two in the morning.
The switch on the wall beside the outdoor unit. It gets switched off and forgotten.
Month and year is plenty. “Never, as far as I know” is a real answer and worth writing down.

What this card is and is not
- It is yoursNothing you type leaves your browser. There is no endpoint behind this page, nothing is saved, and closing the tab erases it. Print it or write it down.
- It is not a warranty documentWarranty terms come from the manufacturer and from whoever installed the system. This card helps you make a claim; it does not make one.
- It is not a service historyOne line saying when it was last looked at is a start. A real history is what a maintenance file accumulates over years, and that is what a plan is for.
- It does not need usFill it in, print it, put it in a drawer, and you are ahead of most households in this county whether you ever call us or not.
